I need a web form with PHP, MYSQL database. That will auto insert fields with certain with date, unit number. check box description. I want a admin that can change anything, and be able to add users with passwords, the users can change their own passwords. Admin can change them as well. There will be two separate forms they can choose from upon login. Case Numbers and Accident Numbers. They can at anytime go between each form until they logout etc. Each picture is the two forms. You will see the fields needed. All are TEXT entry except the auto a signed ones. Which might be alittle hard to explain here. I need both of these to be able to generate a report based on UNIT # and a date. Printable and nice looking. Need a header and footer that I can edit to say the title etc.

I wrote most of it myself in 30 mins from a website that generates mysql and php from fields, the part that will be the hardest for me is the auto generation of number and users, search and reports. I did it with Machform. A two page and worked 90% the way I wanted it to. But Ill see how many people I get involved that wants to do it, I may increase it.

2nd page is like the first one, when they hit submit. it will show a button, with a code, that is like this. the year of the date, with a -0 then a 4 digit sequence number. Here is a what it needs to be looking like 2018-0-SEQUENCENUMBERHERE, they will see that number generated and it will NEVER be used again. It will show with that number and a Okay button . Then they click Okay, it will take them back to the two forms menu again.

Here is the last part for that page. When they click Submit, it will show in the middle of the page with a Okay Button, above that button it shows 1103032SEQENCENUMBERHERE they have to click Okay and it goes back to the main page that shows the two forms. Ill show a example of the 2nd form in a few.

EXAMPLE: Deputy Number is manually put in. EX: 1103 then the date is 03/2/2018, then a auto sequnce number that will NEVER be used again. so it will look like this: 1103032SEQENCENUMBERHERE
