Instrukce k soutěži

Create an excel data base that tracks customer intakes and response to include: gender, birthdate, name, township, services requested, date of request, date of home visit, start of service, name of person who took intake, name of person who did home visit, and comments. Also create a pivot table on a second excel tab with aggregate data in tables and charts that looks good for printing in report style.
I need it to be an easy and clean intake form with the ability to manipulate data. Logo is attached for the report

    1. Would the database be an Excel spreadsheet?
    2. The data entry in this database would be through a form,
    also in excel?
    3. In the end we would have 3 tabs as follows:
    3.1 Guide with the form
    3.2 Tab with data entered in table format
    3.3 Guide with the graphics

    The data entry process would be as described below:
    1. User filling out the form (Guide 1) with
    automatic insertion in Tab 2
    2. Graphing Analysis (Guide 3)

    So as i understood i need to create these columns > gender, birthdate, name, township, services requested, date of request, date of home visit, start of service, name of person who took intake, name of person who did home visit, and comments. Then also need to put pivot table on this data and show it in a proper way also want me to use chart to show numbers.
    What am i going to do with Logo? So 1st i rephrased it so you can clear it that i understood it right then also please help me with this logo by explaining what is the exact use of this logo here. Thanks
