PLEASE SUBMIT TWO PAGES - A title page and a quiz page

We would like to have a pdf booklet designed like the one attached to give to prospects in exchange for taking a quiz or for signing up for our email newsletter. What YOU need to submit to the contest is a sample title page and a page with a quiz - the same idea as pages 1 and 2 of the attached pdf.

Once the winner is chosen, we will hire you to design the complete booklet as well as several other booklets throughout the year.

Print this 10-question quiz and hand it out at a
staff meeting. Once you share the correct answers,
ask teachers to develop one way they can counteract
each quiz fact during their classroom day.
Answer Key: Make up answers, we'll edit later.

No need for a link to an infographic.
