Website MySQL database

Create a MySQL database with accompanying html pages and php code for people to upload Microsoft Word documents to my website. The UPLOAD page must have fields where the users can input important decriptive information about the file, such as FILE NAME, CATEGORY, DESCRIPTION, KEYWORDS, AGE GROUP, USER'S NAME, EMAIL ADDRESS, etc. IF the file is not a Microsoft Word document it should not be accepted. Once uploaded, an email should be sent to my address to inform me. A link should be included in the email to the CHECKING page on the website. On the CHECKING page there should all the pending documents. For each pending document there should be a link where I can download the document. There should also be an ACCEPT button and a DECLINE button on the page. If the ACCEPT button is clicked then (after an 'ARE YOU SURE?' message) an email is sent to the user to inform them of the acceptance and a STATUS field should be populated with ACCEPTED. If the DECLINED button is chosen then (after an 'ARE YOU SURE?' message) an email is sent to the user to inform them, and the file should be deleted from the database. On the DOWNLOAD page(s) the content of the database should be presented with a Pager to ensure that the list is not too long. There should be a DOC link with which the person can download the document in .doc format and a PDF link with which the person can download the document in .pdf format. (When the PDF link is clicked then the .doc file should be converted to .pdf using the appropriate PHP protocol, and then downloaded to the user) There should also be a RATE THIS FILE facility on the DOWNLOAD page where users can give the file a 0-5 star rating. That current rating should always be displayed. There should also be a search facility at the top of the DOWNLOAD page. There should be one text box where the user can search the FILE NAME, DESCRIPTION and KEYWORD fields. There should also be drop-down menus to filter for CATEGORY and AGE GROUP. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
