Create one page in prestashop

I want to Create a page like [login to view URL] How the page [login to view URL] works. The website sells mattress and other beddings. The customer wants a page where anyone can make a test to customize its own mattress by choosing some options. The example website ([login to view URL]) works in this way --- step 1: how old are you? --- step 2: how tall are you? --- step 3: how heavy are you? --- step 4: how do you like sleeping? --- step 5: what mattress consistency do you prefer? --- step 6: have you any disease? After these steps the system must redirect to a "search result page" with the mattress that correspond to the results of the questioners. My customer wants an upgrade in back office products managing page for example: for each mattress he wants to insert an age for the first question (eg. from 20 to 30 years old) for each mattress he wants to insert some centimetres for the second question (eg. from 160 to 170 cm) and so on. You can add these fields using the "attributes and its values" provided by prestashop.
