A survey webpage (over 2 pages)

Hi, We need a 2 page survey created by a good designer, and a thank you page. NEW: Please see additional notes at end of project. Some notes before you make a bid: 1. I only work with developers directly, no intermediaries please 2. No spam bids. If you don't have time to respond directly to this project, please don't. 3. Please show me your best design work to date. I am particularly interested in sites which combine both quality graphics AND good usability 4. I value reliability above all. If you cannot be in regular daily communication, again, please pass this project by. After all this, I am looking for a top notch designer I can continue to work with.
