Convert a form built on Excel into a web form that outputs a PDF

We have delivered many Wordpress sites and have coded custom plugins/ shortcodes. We have also coded webforms in PHP to generate PDF output. I have studied the excel form and the requirement. I suggest we deliver Wordpress shortcode. As for UI, we can use Materialise CSS if the theme uses it else prefer Bootstrap. I need to know if the charges for each field need to be managed in admin and on entering quantity, the totals show? The fields can be fixed? This can be built using Wordpress shortcode. As you are a good technical guy you know Bootstrap is good to built out this form so it will be responsive on mobile. I see in attachment that there number of person but not unit price so there should be unit price column and final and grand total price should automatically calculate through jquery. Calculation will be all done through jquery and after filling all details their will be a button at bottom export as pdf. On clicking it will export a pdf file similar to filled form with grand total. Please let me know if my specifications matches your needs.
