Create a custom email in constant contact to match a printed renewal letter

We need someone to create a custom email template using the HTML editor in Constant Contact. It needs to be similar to the attached sample. It also needs to be able to bring in data from the Constant Contact Custom Fields that we have defined for certain data. The email needs to have two buttons to select a one year or two year renewal using the paypal generated button code as sampled below.
