PHP MySQL Simple Search with Barcode Scanner via Laptop and Mobile Devices

I need a simple website with search box that searches a database via barcode Webcam or Mobile Camera via current browsers, such as Safari, Chrome, Edge, and Firefox (no need to worry about older browser versions), for Employee ID number and displays following information: Employee ID First Name Last Name Beginning Date End Date *Need a separate File with Database Information that I will enter our information when we go live. That way you can test locally on your equipment and not need to log into our local system. I'm aware that I will need to enter dbname, dbuser, dbpass of our system. (You can use whatever variable names you typically use and I'll just replace with our information.)
