Web based Responsive Patient Registration System

Project: Patient Registration System Discription This system needs to be a secure web based fully responsive / Mobiel ready patient registration system. It should have 2 types of users. Read only users and users who can edit/modify. It should work with all mayor browsers and on PC, Tablets, phones etc. (Responsive). Fields required: Name Last Name Date of birth Address District Telephone Number Email Address Weight Body Fat Obese (This should be a field where numbers can be filled in e.g. 7.2 or 7) The following 3 fields will need to be tracked over 6 months. 1x monthly. So each month a value/number needs to be filled in. Need to be able to track patient for 6 months Blood Sugar (This is a value/number that needs to be fille in e.g. 7.2 or 8) Cholesterol (This is a value/number that needs to be fille in e.g. 7.2 or 8) Blood Pressure (This field 2 values need to be filled in with slash in between e.g. 120/110) Comment (This needs to be a large text box when comments can be added for that patient) The system will also need the following buttons: Send Patient info Through email (The patient info has to be sent to others for viewing in a one sheet PDF or similar) Contact Patient (This button needs to send an email directly to the Patient) Print Patient info (The same information will also need to be available for print) The program should have the option to create a new patient, delete a new patient and modify a patient. The program should also have an “Attach file” option, where files can be attached. E.g. Blood test results or any other digital file. There should also be a search option. The search option needs to be able to search all the data. For example: Search under name or last name or age. Search for all patients above a certain age in a certain district etc. So these search options and maybe a small reporting feature. (Ideas are welcome)
