We need a solution to manage multiple field teams (time logging, GPS point capture, client allocation, form scanning, updates, verification of data) - in essence:
• To develop a mobile application to manage lists of contacts to be screened per field worker
• Ability to capture a scan of a screening tool (as well as a nutritional referral tool & TB defaulter tracing tool)
• The image of the screening tool to be OCR’d and the ability for the field worker to verify the form per field to confirm/correct the OCR’d information in the mobile application
• The data to be centrally available for further verification by multiple levels of users
• The contacts that are referred to clinics will have additional linked information that will be updated as they go through the process of further testing and ultimately treatment
• The data to be centrally available to pull a variety of reports
• Possible technologies that could be proposed are Android, CamScanner, ABBYYFlexiCapture, Power BI or any other solution as recommended by the respondent that will solve the M&E data capture, verification workflow and reporting requirements

I have added a PDF version of one of the forms - the form would be filled in by the field worker during their interaction with the client - this form would then be 'scanned' by taking a photo on their Android phone for OCR processing (uploaded to a central OCR server) (an alternative is scanning the form at the field office if the phones don't provide high enough quality scans for the OCR). They would then verify the OCR data on their phone application, field by field (or groups of fields). Supervisors would verify the data on a web application (would need to see the scanned form at the same time).
It is our understanding that we might need to additionally pay for licenses for the OCR processing software - the integration (not 3rd partly licensing) is expected to be part of this project. We would need recommendations on which OCR software - we have looked at ABBYY, but we are open to recommendations.
I have added two flow diagrams to try and help present the user interaction with the system at the different levels.
The CPC Data Source Flow Diagram shows the flow diagram for the TB Screening tool and the link with the clinic systems. Please note, the clinic systems that we are interfacing with are manual paper based systems - they require manual update on our system. The only exception is the NHLS (National Health Laboratory Services) - there is a possibility that we will be able to send them the barcodes of the tests that have been sent to them - and then get electronic feedback from them if the result was positive or negative.
The M&E Data Source Flow Diagram tries to represent the full flow of data between Supervisor & Coordinator / Field Staff / M&E Verification for all 4 processes: TB Screening / TB Defaulter Tracing / Vulnerable patients for nutrition support / Mobilising registers.
In terms of OCR volumes - our target is approximately 12,000 to 15,000 screenings per month for 18 months.
I have had some clarification question on the different status settings as a contact moves through the process as well as a request for some wire diagrams to help represent the app.
I have attached a spreadsheet that has 4 sheets (orange) with some data elements that might help clarify the data structure (this is not a database design).
I also have 5 sheets (green) with some elements of the data that would be needed in some of the Android App screens (this is by no means a comprehensive set of wire diagrams - but should help in visualising the main elements of the process flow).
I also have 8 sheets (red) with some potential screens needed in the web app (again, far from comprehensive but should help).
If one goes with the ABBYY Flexicapture engine, it allows for each field on the Screening form that has been OCR'd to have the text and image of each field made available in an export/database. See http://www.capturedocs.com/thread/how-to-get-field-image-for-table-element/ This means that the user verification on the Mobile App and on the Web could be a combination of image with text below it at the same spacing to make a visual check (and edit) quick and easy. I.e. it would not be an image of the form with fields next to it, but rather cuts of the image per field as extracted by ABBYY from the form with the text below it on a long scroll list.

From a technology perspective, if one goes with ABBYY then we would need a hosted Windows server infrastructure - it would accordingly then make sense that whatever database and web technology is suggested for the web & mobile app runs on the same server. We would also like to have the ability to connect Microsoft Power BI to the database for easy custom reporting/dashboards etc. which could also be called from within the web interface.
