I need a front page for a website built in WordPress, and uploaded to my site.

Minimum requirements: (SEE ATTACHED EXAMPLE "landing page.png")
-Site logo
-email capture space (and corresponding database)
-gallery of images in background
-mobile responsive
-can swap out images in background individually (so it's not one big large image)

To start with, my images are uploaded to OnceUponAPassport.com and the EXAMPLE IS ATTACHED ("landing page.png") of what I was envisioning.

The database ideally would capture:
-email (mandatory)
-one clarifying question (not mandatory)
-time and date stamp
-available marketing related data (time zone, country, browser, if on mobile or desktop, ios, etc)

Please let me know
a) an image of your final vision
b) which parts would be editable in wordpress after it's built (the one clarifying question, the images, etc)
c) how the database access will work

I am open to your additional ideas/past examples of work as well.
