Most "Scan the QR to see...." infographics make use of an image where user holds the phone to see the QR in camera. Seems you are not so familiar with this concept.

I rated this design a bit low because it relies too much on text. We need this to be operational in many languages so it must be more of a graphic description and less of a text description.

Nice use of the palette :) What's that strange Smartphone? It would help understanding if using a "real" device in the picture.
