excel surgical template

I book patients for surgery. I have a sample template but feel I am having to retype a lot of the same headings over and over.
1) if possible If the headings can freeze since hundreds of pts get scheduled
2)when inserting new row of the templates can automatically be in the rows so I can fill the information
3)has to fit in landscape view for printout
4) space above the title headings to place misc info such as important phone #s, logos, etc
Bonus: modify the template as needed to facilitate retrospective research: i.e. how many patients had x procedure this year.?

Certain things to know: What will be the page size? A4? Do you want to print the list in landscape or you want template for each patient( then a .net application will help)?
