website that generates custom paypal invoice

I need a very small website that includes a form for the payment page. The buyer should be checking out with PayPal but the form will ultimate generate an invoice to the user's email address for the buyer to pay with paypal from the email.

Another important feature of this page is that I want the page to block any kind of VPN or Proxy. Also it should display the IP address on the page as to warn the user not to commit fraud.

The form should include:

--A (4 upload picture attachments)

1) upload picture of identification front ID

2) upload back picture of identification

3) Take a Selfie holding ID

4) Selfie holding the hand written note

--B (enter text input)

5)Provide phone number linked to paypal account

6) Enter Trading ID

6a) Enter Username trading with

7) Enter amount to be paid.

8) Enter email address

9) last 4 digits of card or bank account on phone number associated with Paypal account

--C) Generates the Paypal invoice which includes the Terms and Conditions and also provides:

9) Puts the uploaded images in the invoice as a link and includes IP Address ip address as well as all of the text entered in by the buyer in the form.

10) sends to my email for review with first and last name in subject line .

The initial home page will just be a simple page that makes you select a Payment type. When they click 'paypal' it will generate the new page that you will create to make the payment.
