Request Update Form

Resource Database needs "request update form" to utilize community knowledge. Form to be programmed with the following features:

- parse the changes and post them as a text comments for site administrator sortable by time and resource name

- expandable/collapsible content based on the category

- captcha protection

- confirmation notice after subscription

Project also requires the update to existing forms
