OK I need some forms and have the results by email and into a MySQl database with an admin like you completed for [url removed, login to view] for me.

Here is where I need assistance:

1. I need a working copy of the "Quote" page found on a competitors site at [url removed, login to view] poolsan [url removed, login to view] Would like a basic captcha included

a) I need the results sent by email in a .txt format and also logged in to a database with a web based admin. I would need all the fields form this sample form plus an extra few fields in the admin like:

replied to (radio button Y/N)

handled by (field for name)

follow up (field for date)

2. Contact us page like sample at: [url removed, login to view] [url removed, login to view] all these results should go into the PHP list that are entered and an email sent of the results in a .txt format. Would like a basic captcha included

3. Job opportunities page like sample at: [url removed, login to view] [url removed, login to view] but I do not requires the section "Area you prefer to work:" The results of this form only need to be received by email.

4. Warranty Request form like sample fields found at: [url removed, login to view]
