Code 2 simple PHP pages (abset)

We have an existing web-based CRM that we've built. There is a setup process which allows customers to add some basic information before they get to the main sections of the site.

The first 4 steps have been coded already, we need steps 5 and 6 to be coded. Please read through the attached document for full details and instructions and feel free to ask any questions.

You will be given full ftp and cpanel access.
