Create an electronic records system

We want to 3 different Electronic Records Systems.
1) For Health Industry - Health Records (Hospital, Clinics, etc).
2) Educational Department (Elementary School, Middle School, High School, University, etc.)
3) Work Industry (Employment Tracking system)
Basically we want that every ones Electronic Records information goes with them anywhere they go. It needs to be Encrypted and secured (similar to Bitcoin secure) so it wont get hacked. It needs to be constantly communicating with each system so that any changes on the record its updated ASAP. It needs to be able to upload Images, notes, etc. It needs to run off from a secure web.
