Need template for Internet Marketing Business site

I need a template that pretty much copies the following site:

Of course Im going to be adding my own content and images however i want the above site to be used as a site to copy from for YOUR design for my site.

I need the form at the bottom just like in the example page that will collect the users name and email address and then send an email to me.

You can use an already programmed script for this , no doubt. Therefore i expect the name/email form to work when site is delivered

On my back end i will be using the Infinite Responder autoresponder for my newsletter mailing to the customers who email me via the form on the site

I feel this project is pretty simple which is why I'm opening it as a featured project and wish to keep the bidding to no higher then 50 dollars.

I'm going to be using this template over and over for different ideas. So it needs to be modifiable for different products and concepts.
