Gravity Form

This is a very quick job - need a payment form designed with gravity done and installed in my back end.

I need to design a payment form like this one below:

1. Product - separated and spaced out well

2. Billing Information is separated - I also want us to include a shipping information - some customer information is different. We should create the option to copy if details are the same by use of a check box.

3. Order Information Separated

4. Payment Information Separated

5. Place Order Tab

These are important.

Finally look at the feel and how we used the blue to separate this prior form. I want you to be creative and develop something like I showed you. Make it clean

This form should be one I can duplicate and simply change the product image, product information and product cost.
