My wife is a photographer and designing an online sign up form for a dance studio's picture day. She used one last year that worked well BUT many of the parents had dancers in up to 7 classes! She had them fill out a separate form for each class. We want to be able to have someone indicate each class their child is in on one form and next to the class, the photo package they want to order (including not ordering one at all)

The homepage also features a Machform form which works fine, but when submitted, the page scrolls up and the user cannot see the errors (if any) without the need to scroll down to the form. I need the user to see the errors / result of form submission - so the page needs to scroll down where the form is on submission attempt.

A Wufoo Form that will auto fill a editable PDF form. We do Chinese visas, and the embassy has a China visa application via PDF for people to fill out. Many customers fill out the application wrong. I would like to have wufoo form that customers can fill out, and depending on what they fill out will allow them to the next question, ultimately ending in the perfect completed Chinese visa PDF application. Some NON U.S countries need additional documents required in addition to the PDF application, so depending on what country the customer passport is from, the completed application will produce a check list of additional requirements needed when the customer prints out. I have a website www.UtahLiveBands.com. I hired a person to put some forms on the website; however, these forms are no longer working. He used for the forms. When he first did it, he made custom looking forms that validated with the Wufoo backend; however, Wufoo seems to have since made a bunch of changes to their form backend and it no longer validates.
