Hi there! I need a document formatted so that certain fields can be filled out when we see new patients to provide them with a financial estimate of what their treatment will consist of and cost and what the payment options are. This form will need to have areas that will auto-populate other fields within the document. I have the contents and text of the form filled out, but I cannot figure out how to make certain fields "fillable" so that each time we see a patient, we can quickly fill out their estimate forms without having to do a bunch of numeric calculations and re-typing. I have attached a copy of the main form we use and the areas that need to made to be "fillable" are highlighted. I used a color scheme to show which fillable areas would auto-populate later in the form. There are other documents that need to be made as well, but let's start off with this one.

Greetings! Since calculations are involved, I would recommend implementing it in MS Excel. If MS Excel is used, it would provide great flexibility over time - either when calculations have to be changed or when dealing with changes in Appliances used section. The look and feel would be the same as in Word document.
