Build a web app.

Goal- Build a web app that allows a user to create a form with multiple questions, take inputs from that form, then display the form results in a dashboard and allow the admin user the ability to approve or deny the questions, and if they are denied, add a written note. All code must be written in PhP or Javascript Details below: 1. Build a form builder, that’s allows the user to create questions with the following format. 1.a file request 1.b yes/no 1.c select from a list 1.d Boolean question 1.e Text Question- Ie user writes a question, and the answer is text. Each question type should have the ability for the user to set as required or not. 2. Have a form/view/(formnumber) page to render the questions of the form. ( from the form creator, we can create many forms, and we want to be able to add unlimited questions to each form) 3. Users- Super Admin (site Owner)- Admin (vendor users)- Applicants (Client users)- Reviewers (Form Approvers) 4. Super Admin Dashboard- View the following: 1. Button to create Vendor Companies 2. Button To View Existing Vendor Companies 5. Vendor Companies Creation Page Questions: Company Name, Company Phone, Master Admin Name, Master Admin Email, Upon Creation- Email needs to be sent to Master Admin Email 6. Vendor Home Page- Button to Create a new Form ( show the form builder mentioned in step 1) Button to Add a (Reviewer user) 3 buttons across the top of the page ( new forms- Resubmitted forms- Approved Forms) 7. Review Form Page- 1. Show Questions with user submitted responses. 2. Show Files uploaded with file name- and 2 buttons- Approve File- Dissaprove file- if dissaprove file- a notes modal will pop up and the reviewer will submit reason for disapproval, with a confirm disapproval, upon this disapproval, the form submitter (client) will receive an email alerting them to the file disapproval, at that time the client will go back to their submitted form and upload a new file for that field and resubmit the form, which will move the file to the resubmitted category. To expand further, there is a form with request for files a follows. 1. Site Plan 2. Survey 3. Construction Plans. The review sees the results after a client submits the form, they could approve the Site Plans and Survey and Disapprove the Construction plans. So upon resubmittal, the form response would already show that the survey and site plan were approved, and display the reason for disapproval in the 1st instance, so basically there needs to be a running notes field that is added to the form responses.
