Hi, I have an excel form which I want to have converted to a fillable web form. There are some macros that I will need to be translated to js or something else. The fillable parts is colored light and the fields that are filled by scripts are colored dark in the excel. There should be a save button that posts the data to a db. Nice layout is important! I want the form to look very nice! Rearrangement of the parts in the excel is fine as long as it's all there. Will NOT accept any offer without a layout mockup! I've attached the excel.
