QuickTime Video Editing & More

I have a 5 minute + or - long QuickTime Movie that needs to have the following done to it… It needs to have a voiceover added as well as some soft background music (each will be provided). It then needs to be saved into a format that includes controls (pause, stop, start, etc.) to be placed on a CD (this CD should be coded for auto-play and work on PC and Mac formats). The file also needs to be saved so it can be placed on a website in a streaming format that will not be too large for an average visitor to enjoy from a dial-up connection if possible. I would also like this version to have the controls if possible (pause, stop, start, etc.). Please consider all of this in your bid. Please tell me the format you need this in (QuickTime standalone movie is 150 MB. QuickTime ‘normal save’ is only 100 K but has no dependency files). I am new to QuickTime format so please tell me exactly what you need.
