I would like to hire a Coder

I have a standalone project which, depending how successfully it was done and how competent the person was, could be incorporated into my main site at a later date. We use php and MySql languages Please see the attached document 1) I want a one-page screen that allows you to set up a new page like this for each new loan a) it needs to be pre-populated with the information as in the attached document b) it needs to allow you to edit any of this information c) it needs to allow the user to enter new lines or delete existing lines d) when you press submit, it enters all this information into a database (database probably needs one table to list the loans and another table to list the information required for each loan) 2) It then needs to produce a URL that allows a user to view the information but not change it - an easy to remember URL like [login to view URL][BorrowerName]&id=[unique ID such as a number like 123] 3) It then needs to produce a URL that allows a user to edit the comments field only and add new boxes if required but not change the other info - again an easy to remember URL like [login to view URL][BorrowerName]&id=[unique ID such as a number like 123]&edit=1&user=[DWS] When you insert a comment, it adds the date and time that the comment was inserted (in the comment box), and if you insert a new line, then it adds the date and time that the new line was inserted in the comments box 4) We need to be able to create a list of users who are allowed to edit (ie a 3rd table in the mysql database)
