Small ASP/Access db system

I need an Access/ASP driven system that is a simple data entry and display system that works like this. **Page 1**: I need a form that takes data and places it into a database (I need information and commenting so that I can easily add new fields/field types into the form and the add the field in the database in case I need to collect more data in the future). When they click on submit, they are then asked on another page which category they want the record to be grouped into. This should use drop down menus in exactly the same way as page 3 does (see below). **Page 2**: I then need a page that allows administrators to create a category and subcategories of a category. These subcategories should then be listed in a drop-down menu on the form (page 1) so that users can only choose the categories listed and not just enter any text they want. When a user clicks on a category or subcategory they are taken to a page where they can either rename, delete or add another subcategory to it (please see attached file to demonstrate the layout). All categories and subcategories should be allocated a number. Then, when a user fills out the form (page 1), they are asked to specify which category they want the record grouped into by asking for the category number. **Page 3**: I then need a page that allows users to select these categories from a drop-down-menu, then select a subcategory from another menu that appears under the main menu (if there are any subcategories). Then, once the correct category and subcategory is selected, the user clicks on 'Submit' and a list of all of the records that are in that category are listed. The user should then be able to click on the name of the record and bring up a page (**Page 4**) that contains all of the information of that record. This page should have an 'Update' button that when clicked on, allows users to edit all of the fields (**Page 5)** (I will protect this page with NTFS permissions). That is about it. I urgently need this completed by afternoon (GMT) Tuesday 20th July. All code must be commented and each page should have no formatting - text should not be formatted and the form fields should be positioned in a table. I also need brief information about how I can add new fields to the form (page 1) and to the database. Please only bid if you can meet this deadline. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Work with IIS 5.0 and all modern browsers.
