Registration page with email confirmation

Sorry I closed the previous one by mistake. Made some adjustments as well. I need a VERY SIMPLE web form to register users, graphic is already done. FIELDS: a) email (add check if email already exists in the database) b) password + confirmation password (8 characters minimum) c) registration date d) registration confirmed (based on the email confirmation) EMAIL: a) an email should be sent after registration for confirmation (email template and graphics already exists) OTHER: a) [optional] the form should permit users to register with a Facebook account (store email, first name and last name). This is an additional cost to be added in your bids. Not sure I will go for Facebook log in now. RECRUITEMENT PROCESS a) I will shortlist 5-7 people within the day and answer all their questions/send them the pages and email template b) We will review the price if necessary. I am more focused about the quality of the work and the cost than the time to do the work. c) This is only one piece of the website, if I am happy with the result, there is more to come, more complex Please add reference 'SYDNEY' when bidding so I know you read all the requirements and to avoid waist of time.
