Write an iPhone application

We are looking for an app to be built for internal company use. The specs are below. Requirements will be to read some data from a sql database and then also post results of the app to a database as well. Initially we intend this to be an ipad app viewed in landscape mode. but it would be helpful to have it available on other tablets for the future. Here is what we'd like: Viewed in landscape mode Main Page Banner at top: My Jiffy Lube Personalized Service Screen Split in ½ Left side picture placeholder with picture being added from camera (Example – picture of customer’s car) Right side picture image set by administrator (example – picture of Jiffy Lube store) – static Under left picture – Pre Service Banner Three icons Customer Info Customer Preferences Original Estimate Under right picture – Current Service Banner Three icons Report on Conditions Service Notes Estimate / Delivery Customer Info Page: Banner: Customer Information Page Data entry: License Plate Number and License Plate State – Query database and return data below if present Left side of screen: Right Side of Screen Banner: Customer Specifics Banner: Vehicle Specifics First Name Year Last Name Make Address Model City Mileage State Front Tire Pressure Zip Rear Tire Pressure E-Mail Previous Oil Mobil Phone Land Line Customer Preferences Page Banner: Customer Preferences Note pad page. Original Estimate Page Banner: Original Estimate Picture holder to snap picture of estimate using camera. Data entry point to enter dollar amount of original estimate Report on Conditions Page: This page will be a grid of nine icons representing different items checked during service. Each icon will link to a new page that will have details of the item (described within) Each of the icons will be overlayed with green check if the linked item is OK, a red exclamation point if the item is recommended to be replaced and a recycle logo if the item is chosen to be replaced along with the price. Underlying product pages: Banner: (title of particular product) Screen cut in half Left side placeholder for picture of customers particular item snapped by camera Right side picture will be a picture of a new product (static) Under the left picture will be four icons. A good icon with a green check mark (carries up to the report on conditions screen) A Recommend Replacement icon with an data entry for the price A replace icon with the recycle logo An N/A button (meaning Non Applicable) Under the right picture will be a list of benefits of the particular product Notes Page: Banner: Special Service Notes Note pad page Estimate / Delivery Page Banner: Final Revised Estimate This page will summarize the totals of the original estimate entered on the original estimate page Add any items from the report on conditions recommendations (list name of product and price) This page will also include a signature capture spot as well as letting customer choose how they’d like to receive the estimate … e-mail or print. The app should then e-mail or print a formatted estimate to the customer along with other information provided to perform with CA law. Finally – the information should be uploaded to a database for storage (all entered data as well as pictures) and one submitted reset for the next use.
