61690 Simple Form to Database Script

OUr company needs a very simple basic script. Below are the instructions on what needs to be complete for this script. The script needs to be complete within three days and in my personal opinion it shouldn't take more than a few hours. [login to view URL]~loan/[login to view URL] The script should save all of the information that were inserted in fields on the [login to view URL] in a mysql database. When the information is saved in the database, the script should then display one of the five pages using the information the user entered for the loan type they want. [login to view URL]~loan/[login to view URL] The script should display one of the five results pages based on the information the user entered on the [login to view URL] page. [login to view URL] should be displayed if the user selected to search for a personal loan, [login to view URL] should be displayed if the user selected to search for a personal loan, and you should get the point on what other pages should be displayed if they select a different loan type. [login to view URL]~loan/[login to view URL] The user must enter a valid email address. After the click the button the script should add their email address to the bottom of the [login to view URL] file and be taken to unsubscribe2.hp.
