Script to change a user's http referrer and user agent

Hi, I need a web based script that can redirect a user to another domain (example [login to view URL]) but the script needs to also change the user's HTTP referrer (meaning change the information about the referrer page) and to change the useragent. Example: the script may be put on [login to view URL], and when a user is redirected to [login to view URL], [login to view URL] should see the user as coming from itself (meaning the referrer will be [login to view URL]) instead of domainA.com. If the script can put a different useragent, that would be a bonus as well. That's all - thank you.
