Dynamic database contents display script

Hello I'm looking for an experienced html/mysql/php programmer (at least I think that these will be needed). I need someone to make a script on page for my client. All the page design is ready, I just need the script to display values from MySQL database (very simple one - only a month and % value for it). Right now it is static, but must be loaded dynamically on every page access. This should be flexible script, allowing to be used for at least a few years (let's say 2020, but this page will definitely not last that long it this form...). I also need some easy way to insert values into database, best solution is simple, 'idiot-safe' html page. The easiest and most user-friendly solution wins. I don't need something complicated, it has to be as easy as possible.
