181327 simple data submit PHP script

Hi! I'd like to order a simple PHP script, that would allow me to fill MySQL database (one table) in a certain format via a simple web form (record by record). The form should look like this: Field_name1: -------textinputarea------- Field_name2: -------textinputarea------- ..... Field_name3: -------textinputarea------- Submit_Button The form should also have a link to a page that displays a table with all existing records of "new_ss_sites" table, and allows to edit and delete each record. Attach the exact format of database. P.S. Fields "LeftAd" and "RightAd" are supposed to store Adsense code, which is several lines long (with line breaks). So the format of these 2 fields should probably be different from what you'll see in the attach file. Thanks!
