562339 Create a form to schedule appointments

I want to provide customers with a very simple 'Book now' form that will need basic info of the customer a little calendar with time and the choice of service they want. This form should work along with Paypal, to process the payments and send both the client and me an email confirming the booking with all info about it. This is to be used with a WordPress template. To give you an example: [login to view URL] The form has to have name (first and last) phone address type of service from a drop down and check boxes for additional services. discount box (not needed but helpful), and also an option to pay later or via credit card just like in the example. I know basic PHP and would probably know how to do this (looking for tutorials and what not), but I just have no time.
