Downloadable content landing page and contact forms

Project timeframe - 1 week. Part #1 Design a great landing page template to be used for downloadable content such as "download the checklist" or download the free whitepaper". Must have inbuilt form to capture the user's data before download for lead generation. Additionally the freelancer must be able to provide a small how-to guide for how to use this template for ongoing downloadable content and also how to attach tracking code to track visits and downloads. Part#2 Design contact forms to be used on various sections of the website. Must provide some designs to consider and also a how-to guide of how to add the same style of contact forms in future.
