MaxMind MinFraud Integration with Simple Hosted Form

Need to create a simple registration form that includes the basic fields: -First Name -Last Name -Email -Phone # -Address The form will include the regular Maxmind Javascript code to detect device ID [login to view URL] + the MaxMind's MinFraud service API integration.
