I am trying to get an app/software which will eliminate writing work. I have some forms that I need to fill on regular basis. I want to convert those forms to computerized form where my clients can sign the form on an ipad and i can later type and fill the information rather than writing it. Attached is a sample blank form. (It is very important for me to print out the form in the exact same format as the original just filled with a signature at the bottom)

Hello, I did such type of applications but bigger ones. And I know how to realize it. To realize it I have to make a business-analysis and a system analysis first. Then I should to elaborate a structure of expected application - server side part, user side part, an interface between these parts. Then I need to create a relation database structure. Then I have to design a prototype of a Graphical User Interface. And only after all these steps I can elaborate a source code of the server side and user side of the application.
